Relapse Prevention Case Study, Oakenhead Resource Centre.
Background information regarding referral:
A young man (Mr. X) on referral was 17 years of age. He had been drinking from the age of 15 year. Mr. X had since September 2007 been a daily dependent drinker. Mr. X drinks daily 16 cans of 5% lager daily. Mr. X lived within the family home, his father also a dependent drinker.
Social Worker Role:
To commission service and allocate funding for placement into Detox and/or Rehabilitation Unit within Lancashire.
Resource Centre Support Worker Role:
• Point of contact to access Mr. X away from the home situation
• To motivate, support and facilitate Mr. Xs transition to the point of admission into residential rehab
• Support Mr. X to attend the Detox Assessment at the Rehab Unit prior to his admission.
• Intervention with Personal Care.
• Support Mr. X to access the IT Suite within the Centre.
Other working Partnerships
• Social Worker Substance Misuse.
• Early Break – Young people drug and alcohol service.
Effect on individual from attending the Centre:
• Personal hygiene improved
• General appearance improved
• Mr. X attended the Centre twice a week and during that time he became controlled regarding his intake of alcohol.
• Able to access the internet to gain knowledge and information on the various rehab units available within Lancashire.
Expectation (IAP Goal) from client:
• Rehab assessment;
• Admission to a Rehab unit;
• Improve personal hygiene;
• Access IT facilities within Resource
Client`s identified strengths:
• Commitment to Rehab process
• Commitment to give up drinking
• Commitment to attend Resource Centre regularly.
How the client sees the future:
• Mr. X was committed to attend a unit and to stop drinking.
• Mr. X was considering returning to full time education.
• Mr. X considered his future to be alcohol free.
Outcome
• Mr. X was referred, had a successful assessment and was admitted to Bennett House, Crumpsall Hospital, Manchester.
